We have an exciting opportunity for a Carer Support Adviser to join our Virtual Wards project in Mid Essex. You will be working alongside NHS colleagues in a multi-disciplinary approach to supporting carers who have a family member on a virtual ward.
- Carer Support Adviser- (Virtual Wards) Mid Essex**Salary**: £22,571 per annum

Contract: Fixed term until September 2024

Hours: 37 hours per Week
- Location: Mid Essex - Flexible - Hybrid/RemoteVirtual wards (also known as hospital at home) allow patients to get the care they need at home rather than being in hospital. As a Carers Support Adviser, you will be part of our Mid Essex community team, helping to improve the lives of carers through your work.

This role will include working alongside the virtual wards teams to identify carers. You will be managing conversations with carers to identify needs, providing information, advice and well-being support, running training sessions on the Significant Care tool and undertaking pro-active identification of carers through partnership working.

To be successful in this role, you will have experience working with adults either in a 1:1 or group setting, and a working knowledge of health and social care, particularly in relation to adults and experience working in partnership with community and statutory organisations. You will have excellent communication and interpersonal skills and experience delivering training to small groups.
