Waltham Forest Community Adults Rapid Response team is an expanding 24 hour 7 days a week admissions avoidance service working with both primary care, and acute sector partners in the London Borough of Waltham Forest to support and deliver treatment to patients at home.

We are currently looking for nurse/allied health professional advanced clinical practitioners who is motivated and committed with effective leadership/management skills. The post holder will have significant community urgent care response experience.

This role will require the post holder to work flexible including day and night shifts to support the team operational hours.

The Operational Lead will be responsible for the professional and clinical leadership, line management and development of the services ensuring delivery of safe, effective and efficient services within an agreed locality.

**The post holder will be responsible for**:
1. To regularly review resources and take a lead in developing skill mix within the team
2. Implementing service strategies, including workforce strategies to manage the increasing demand for community services
3. Maintaining high quality, patient-focused care for their service
4. Achieving and maintaining quality care through the delivery of evidenced based/research based practice
5. Ensuring governance for the service is embedded
6. Implementation of audit
7. Managing the budget ensuring a balanced financial position for the individual cost centres

COVID-19 Vaccination

Probationary Period

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

Maintain clinical input - 80% (management), 20% (clinical) Key Responsibilities:
1. Work with peers and other managers within the services across NELFT, and external partners to ensure that service delivery is effective and efficient
2. Monitor national and local guidance and policy relating to community services and participate in the development of local policies to ensure consistency with national directors and local frameworks
3. Manage the service in line with agreed financial and quality objectives and be responsible for taking corrective action as required to ensure the efficient and effective management of resources to deliver the desired quality and quantity of service
4. Ensure that all targets for Unplanned Care are communicated, understood and achieved
5. Develop a performance management framework
6. Develop an agreed data set to satisfy the needs of activity reporting and quality reporting and to facilitate clinical review/audit of care
7. Actively contribute to the production of a monthly performance report for the managed areas and analyse monthly activity information for services provided, taking corrective action to maintain activity targets
8. Develop and maintain a system to monitor professional registration of staff
9. Deputise for the Head of Unplanned Care
10. Provide cross cover for other localities in the absence of peers
11. Responsible for ensuring staff compliance with Trust policies and standards
12. Responsible for ensuring that overall day-to-day management arrangements are in place to ensure effective health care delivery to patient/users of services
13. To work with the Assistant Director, Head of Integrated Community Teams, Medicines Management and Practice Development Staff to ensure that all non-medical prescribers are practicing with local agreed prescribing policies and support the monitoring of Nurse Prescribing activities
