Borders Additional Needs Group (BANG) is an OSCR registered Charity dedicated to supporting families living in the Scottish Borders who have children with additional support needs including autism.BANG provides information on resources, benefits and training and works in partnership with the t=other stakeholders.

**“Branching Out Transitional Youth Project” and “Branching In”**

The Branching Out project, delivered by Borders Additional Needs Group SCIO, forms a Transitional Youth programme for 16-24 year old young people with an Additional Support Need, including Autism and Learning Disability. This Transitional Youth Programme works alongside our pre youth provision ‘Branching In’ for youths aged 11-15 years with an Additional Support Need and is funded by Young Start. ‘Branching In’ is held on a Thursday evening in the Focus Centre, Galashiels. The project will create a Young Person profile for each of its members capturing goals, strengths and person centred information which will be used as a transitional resource tool, recording their achievements, training, volunteering and life skills as they work towards the transition into adulthood.

Branching Out Transitional Weekly Group is held on a Friday afternoon based at the Focus Centre, Galashiels where the young people participate in the running of the group, learn budgeting skills, fundraising, attend enhanced ASN employment programmes, youth training access to youth qualifications, social activities and independent living skills. The Focus Centre is close to local bus links to encourage more independent travel although transport can be provided if necessary.

**Purpose**

This role is an exciting new opportunity for a motivated and enthusiastic individual to use their creativity, skills, and experience to make a difference to lives of ASN young people in the Scottish Borders. Your role will be within Branching Out 16-24-Year-old group to support with the delivery of pilot Transition Work with ASN Youths to support the aims of Branching Out and transitioning into adulthood.

1. To deliver the project in line with Transitional Youth Coordinator to achieve the outcomes set out in the funding brief.

2. Work collaboratively with Branching Out staff and professionals to connect them into community, health, employment support.

3. To support the delivery of 1-1 transitional support to young people within the project who require more support to ensure they have fulfilling lives.

**4. ** To work with Transitional Project Coordinator to Co-Produce Young Person Profile to ensure their unmet needs are identified, their goals are identified enabling young people to self-advocate empowering them to using a resource that communicates their needs and goals**.**

5. To support in the delivery of accredited training, life skills and other transitional support that benefits the young person.

**Responsibilities**

1. To develop and deliver a youth transition support programme offering 1-1 support for ASN Young people.

2. To work with Transitional Project Coordinator to monitor, collect data and reports.

3. To source transitional resources and support for ASN Young people.

4. To support in the delivery of accredited training on transitional aspects of ASN Young person in conjunction with Branching Out team

5. To support times of transition for young people within the group and connect with their parents to provide a whole family support model.

6. To be responsible for gathering and recording case notes and for the young people.

7. Working with Young person to create a Young Person Profile supported transition document.

8. To be familiar with other services available for ASN young people in the Scottish Borders

9. To be prepared to undertake additional training as required by the project.

10. To work closely with the Youth Project Coordinator and ASN Family Liaison worker as partners in delivering and reporting on the project aims and outcomes.
