**Purpose of the Post**

CRT Engage is a person centred programme that delivers bespoke 1-1 support to individuals experiencing challenges that impact on their health and wellbeing. These challenges typically include; anxiety; depression; social isolation; low self-esteem; low motivation; poor physical and mental health; lack of family and community support; debt; low skills; low income; and unemployment. It also builds networks in communities to increase participation in social groups and activities that improve health. CRT Engage delivers in the Five Towns area of Wakefield and also in
Meir, Stoke on Trent. This role will involve remote working and occasional visits with CRT Engage
Meir.

The purpose of the role is to provide one to one support to people who are looking to overcome barriers to improve their health and wellbeing. This will involve consultation with individuals on healthy nutrition, lifestyle and dietary choices and the creation of meal plans for those who want to lose weight, add muscle or achieve other nutrition-related goals.

The role also involves supporting individuals who have pre-existing medical conditions such as IBS and diabetes and undertaking analysis of their diet with the use of nutrition software. CRT Engage works with a range of stakeholders in the community and there will be a requirement to liaise with referral organisations and maintain positive relationships with partners. The role also requires close working relationships with the Health and Wellbeing Coaches who are part of the CRT Engage team and to work on an outreach basis within the community as necessary, delivering activities such as events, one to ones and workshops that respond to the challenges that exist in former mining communities. CRT Engage establishes and delivers group activities, either through the direct delivery of sessions or enabling local groups to create new provision. This role will be required to support this aspect where appropriate, and also to link people to other services that meet their needs.

CRT Engage is an important strategic programme and contributes towards the delivery of the CRT’s
Strategy and England Delivery Plan. The role is an essential part of this programme and will be required to maintain quality standards, comply with our internal systems and processes and work with the Development Manager - CRT Engage to review the effectiveness of the programme in response to community need.
