Permanent contract - Full time, Mon -Fri, 35 hours per week (Fridays are mostly work from home).

Salary is **£24,400** during probation period and after this it will be **£25,000.**

The HWCs will be employed by Brent Mencap and work in a Brent Primary Care Network (PCN) to promote and facilitate the development of a person-centred approach to behavioural and life style change. You will act as a guide to patients in addressing health issues and the need to improve health.

PCNs are a key aspect of the NHS Long Term Plan, with general practices being a part of the network. PCNs, and the structures they provide, allow services to be developed locally, in response to the needs of the patients in their area.

Many patients are living with conditions such as diabetes, heart disease and asthma and mental health issues so getting help from our local health services is more important than ever. That’s why many GP practices are working with each other and with community mental health, social care, pharmacy, hospitals, and voluntary services in their local area. These are called primary care networks. Working together means they can have a bigger teams, sharing information and proving better services to patients.

As a SPLW you must be a good listener, have time for people and be committed to supporting local communities to care for each other. You will have great interpersonal skills in supporting people. The post holder will work with a diverse range of people from different cultural and social backgrounds. The ability to work confidently and effectively in a diverse, and sometimes challenging environment is essential.

**As a HWC you will**:

- Promote and facilitate the development of a person centred approach to behavioural and life style change
- Act as a guide to patients in addressing health issues and the need to improve health
- Work with the clinical directors of your allocated PCN to development the PCN health improvement plan
- Develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multidisciplinary teams to holistically support patients’ wider health and wellbeing, public health, and contributing to the reduction of health inequalities.
- Provide education and training to PCN staff, supporting them to improve their skills and understanding of personalised care, behavioural approaches and ensuring consistency in the follow up of people’s goals.
- Raise awareness within the PCN of shared-decision making and decision support tools.
- Coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives
- Working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing
- Increasing patient motivation to self-manage and adopt healthy behaviours
- Work with patients with lower activation scores to understand their level of knowledge, skills and confidence (their “Activation” level), when engaging with their health and well-being and subsequently supporting them in shared decision-making conversations
- Participate in the activities of the PCN and practices including key forum meeting such as the Nurse Forum, Practice Manager Forum and PCN meetings.
