An exciting opportunity has arisen for a qualified Advanced Clinical Practitioner to join our high profile palliative care team at Nottingham University Hospitals. Applications will also be welcomed from current year 3 trainee advanced clinical practitioners with imminent qualification pending. This post can either be a fixed term contract or secondment for 12 months with current manager agreement and there is an expectation of further funding upon the success of this initiative. Job share will also be considered.

Main duties of the job

We are seeking an enthusiastic and dynamic qualified ACP to join our motivated team and lead the development and implementation of our new virtual wards. This will include a virtual inpatients and day therapy ward allowing patients to get the care they need safely and conveniently at home rather than being in hospital. These roles also provide an opportunity to work within outpatient clinics and provide telephone advice and support to inpatient areas through the palliative care outreach services. This role will encompass all four pillars of advanced clinical practice including:

- The delivery of education and support to all multi-disciplinary team members
- Development and undertaking of clinical audit and research relevant to palliative care
- Service improvement, evaluation and development implementing our new virtual wards
- Strategic and clinical decision making in the development and delivery of local palliative care services and end of Life initiatives

Palliative care consists of a specialist multidisciplinary team providing holistic palliative care to patients experiencing a life limiting condition and their families. Services are provided within Hayward House for inpatients and day therapy and also the wider population including inpatients within NUH and the wider community offering advice and networking with primary care colleagues.

**Qualifications**:
Essential criteria
- MSc in advanced practice or equivalent (or imminent)
- Registered NMP
- evidence of teaching, supervision and assessment

Desirable criteria
- mentor/assessor/teaching qualification

Experience

- evidence of continued CPD
- palliative care experience

Desirable criteria
- service improvement and development

Knowledge

Essential criteria
- knowledge of virtual wards
- ability to respond to, prioritise and analyse complex health conditions

**Skills**:
Essential criteria
- able to identify learning opportunities and able to organise own learning and development
- communication skills that facilitate building honest and trusting partnerships with patients, families and carers
- ability to work within MDT, often presenting and receiving complex information
