Liaising with District Nurses and Rapid response team Management of the HCA workforce that provide a sitting service for EOL Patients Advance care Planning Communication and coordination with all relevant team members in both NELFT and Barts Health EPIC is a small team that works closely with the all the providers to provide excellent End of life care We offer support to families and Patients at the end stages of Life We complete a training programme to help with development within the role.
