**Social Care Assessor**

£16.02 per hour for working 37.5 hours per week

**As part of our future strategies and our commitment to creating an outstanding service, we are enhancing our service to include the social care aspect within our home. To accomplish this, we are looking for a **hard-working **dedicated experienced **Social **Care Assessor, this person will work closely with the nursing team to ensure an outstanding service is delivered.**

We are looking for someone who works in a strength-based approach, is motivated, flexible, who has high professional standards alongside sound knowledge and experience of current legislation. Experience of undertaking safeguarding enquiries is essential alongside experience of working in complex situations using professional judgement and a range of interventions.

This will involve working across various unique projects such as providing service users the opportunity to enjoy a home they love, in an environment where they feel safe, have friends, and are encouraged or supported to live a healthy and meaningful lifestyle. You will be responsible for working on targeted reviews where you will be expected to undertake a number of assessments and reviews with family per week.

**JOB SUMMARY**

1. Carry out Audits on the following:

- Care Plan Audits across the home
- Investigate Safeguarding issues across the home reporting back to the home manager, and support the nursing team with less complex matters.
- Communicate with social workers
- Undertake social care meetings and reviews
- Undertake in house inspections as part of quality improvement
- Arrange meetings with families for plan of care reviews
- Assist families with any funding issues with the local authorities
- To deal with any complaints and investigate feedback to joint Care Managers

Be able to produce audit reports on all of the above

2. Continue to Develop Audit Systems in conjunction with Care Managers with regards to Quality Assurance within the service.

3. To support the Care Managers with any investigations from incident reports that may arise.

4. To complete any investigations regarding to any complaints, and involvement from the Adult Protection Safeguarding Process with the Care Managers.

5. Out of hours visits as requested by Care Managers

6. Be a reliable source of information for all staff, ensuring that your knowledge is in line with current best practice

7. Supporting and advise the Care Managers in all aspects of day to day running of the Homes.

**Because this is a new and developing role, there may be additional requirements moving forward.**
