An exciting opportunity has arisen for an 18 Week RTT (Referral to Treatment) Patient Tracker to join the 18 Week Team at Harrogate & District NHS Foundation Trust. The post holder will form part of a small team, being responsible for tracking patient pathways within HDFT to support the delivery of the 18 Week Referral to Treatment (RTT) standard.

The post holder will be responsible for identifying patients at varying points in their journey through their 18 week pathways by review of clinical documents from various patient administration systems and recording this information correctly within the 18 Week PTL (Patient Tracking List). To liaise with both internal departments and external Trusts regarding the progression of patient pathways. To assist the relevant directorate to review these patients by providing accurate data and consistent guidance.

Please note, this job is primarily office based, with potential opportunity to work from home one day a week.

**The principle responsibilities of the post are to**:
- To be responsible for tracking all patients within the three Clinical Directorates (Planned and Surgical Care, Long Term and Unscheduled Care and Community & Children's Directorate) to support delivery of the 18 Week Referral to Treatment (RTT) standard
- To identify patients at varying points in their journey through their 18 week pathways from referral to first definitive treatment, by means of undertaking validation and inputting 18 week RTT clock stop and start codes into patient’s pathways as appropriate
- Assisting the relevant directorate to review these patients by providing accurate data and consistent guidance to ensure that all patients are seen and treated within 18 weeks, alongside highlighting themes and trends
- To be responsible for ensuring accurate and timely transfer of patient pathway information for all patients referred to external organisations. There is an emphasis on working efficiently to tight deadlines in order to meet the 18 week standard.
- To develop positive working relationships with staff both within the Trust and externally and work in collaboration to prevent 18 week breaches.
- Providing training to staff members within the Trust to ensure the Patient Access Policy is implemented throughout the Trust
