Senior Integrated Support Assistant

This exciting new role has been developed to support the newly formed Urgent Community Response (UCR) and 'virtual ward' services working across the South Tees locality.

Virtual Wards provide an alternative to hospital admission, for people who are unlikely to benefit from care in an inpatient hospital bed.

Patients on a Virtual Ward would receive appropriate assessments, investigations, individualised planning and treatments in the homes of frailer residents of the North and South Tees locality, exactly as they would currently receive in hospital.

Virtual wards aim to improve patients quality of healthcare, by reducing inpatient healthcare associated events such as delirium, hospital-acquired infections, deconditioning and loss of mobility or falls with injury. In addition to improving patient experience by increasing time spent towards the end of their life in their home, closer to family and friends.

Urgent Community Response is a Nationally mandated model of care where patients who require it are assessed and treated within 2 hours of referral.
