Would you like to be part of a team helping to relieve the pressures on the NHS by supporting those leaving hospital?

This post involves providing a commissioned service as agreed with the NHS at the point of discharge for individuals living in their own homes. It will involve a variety of different task which may include, collection of patients from hospital, helping them to get to appointments, basic food preparation, light cleaning, shopping and collecting prescriptions for them. Successful individuals will be compassionate, organised and highly motivated to provide high levels of support to clients, ensuring that those initial weeks post discharge pave the way to returning to living independently at home.

The post is 30 hours per week working Monday - Friday between 9am - 5pm. This is initially a temporary contract until 30/09/2024, based on the needs of the service there could be a possibility to move to a rolling contract thereafter.

The pay rate is £17,940 per annum
