We are looking for a Health and Housing Coordinator covering two hospitals located in Ashford and Margate. This is a Fixed term Contract to March 2023. Our client provides assistance to vulnerable tenants and people living in our communities who need supportto live well and independently. They provide specialist housing, care and support to people who are socially excluded, disabled, unwell or homeless.

As an integral member of the discharge team for both hospitals, you will provide an effective, timely and robust approach to patient discharge. In this fast-paced environment you will find, investigate and provide resolutions to issues preventing a patientfrom going home safely.

You will at times be required to provide discharge support to associated hospitals, step down bed facilities and other acute hospitals in Kent. The role will also require you to undertake home visits as necessary, actively assessing the safety and suitabilityof the home environment. The role is mainly assisting those that are being discharged from hospital and need support in administrating or arranging their home to be suitable for them.

Skills/experience required
- Demonstrable experience of providing support to vulnerable people either in a community volunteering or professional role.
- Empathetic approach to dealing with customers from a range of diverse backgrounds.
- Knowledge of products, solutions and information on related services to include but not limited to; Welfare benefits, grants home maintenance, housing options, safety within the home, adaptations and equipment.
- Agile, quick-thinking and a proactive problem solver in order to meet tight deadlines.
