Carers Liaison Worker

35 hours per week SJC scale SCP25 £27,926 + 5% employer pension contribution
- To provide advice, information, advocacy and support to adult carers in the Cheviot area
- To provide one-to-one emotional, practical and advocacy support to carersTo provide information and advice
- To assess carers’ needs and complete a Carers Support Plan
- To signpost and refer carers to relevant statutory and voluntary services
- To work within a quality assurance framework and keep accurate and confidential carers records
- To improve the lives of unpaid carers
- To provide access to funding and respite opportunities

Driving license and access to a vehicle desirable - mileage rate paid
Knowledge of Community Care and Carer Legislation is essential
