**Outreach Enablers required in the Hatherleigh and Okehampton areas, must have own transport.**

**Main Aim, Objective, and Responsibility**:
At Made-Well outreach we aim to support and empower individuals to live happy and fulfilled lives in their own homes with links to the wider community. We work in a person-centred way which allows the people we work with to live an autonomous life. We encourage our clients to have a voice and to make informed healthy life-style choices to support their independent living ability.

**How we do this**:
Within this very varied role we aim to support our clients who struggle with daily life, some individuals struggle with their mental health others with their learning ability. We aim to help them live independently by:

- Supporting them to keep a healthy home which is cleaned to a reasonable standard.
- Supporting them to eat well and meet their nutritional needs, making shopping lists, shopping, preparing food, meal planning, and to learn new cookery skills.
- Support them to access the wider community, use community facilities, arrange medical appointments, attend medical appointments, organisation of medication, sign posting where required.
- Support with budgeting and finances, paying bills and organising benefits with them and support with any tenancy matters.
- Offer any emotional support if needed
- Support to stay safe
- Prompts with personal care (No personal care is provided by us)
- Prompts with medication (No medication administration is provided by us)
- Support them to talk to professionals involved with their care, care co-ordinators, social workers, family members, doctors etc
- Complete various administration tasks, such as, keeping accurate records, reading through care plans.
- Communicate effectively with colleagues, management, professionals, family etc.
- This is not an exhaustive list, and many times you may have to use your initiative. However, a member of the management team will be available to confer with at all times. So, you are never alone, we are here to support you.
