Social Care Practitioner

We have an excellent opportunity for a temporary Social Care Practitioner covering maternity leave for 10 months to join our busy, but supportive generic adults, team covering the Derbyshire Dales South area of Derbyshire.
We are based in Ashbourne and our Team consists of Social Workers, OT’s, Social Care Practitioners, a Senior Practitioner, and a Service Manager.
We are a very busy team, working predominantly alongside adults over the age of 18, with the exception of working with younger adults, as they approach adulthood.
We are looking for an experienced practitioner, with excellent knowledge and experience of working with the Care Act (2014), alongside supporting legislation i.e., Mental Capacity Act (2005).
We provide a wide range of services to our community, including Care Assessment, support plans, advocacy, area duty, safeguarding duties to name a few. We are passionate about a strength
- based approaches and outcomes.
In the coming months we are about to embark improved integrated ways of working, with health colleagues taking a collaborative approach in supporting our community.
