Job Title: Support Worker - Children & Young People 16+ Transition Homes

Key Responsibilities:

- Provide practical and emotional support to young adults in a 16+ Transition Home setting.
- Develop and implement care plans that are tailored to meet the individual needs of each young adult.
- Assist young adults with day-to-day tasks, including cooking, cleaning, and budgeting.
- Support young adults with personal care, where necessary.
- Provide advocacy and signposting to appropriate services as required.
- Work in partnership with colleagues, external agencies, and families to ensure young adults receive high-quality care and support.
- Maintain accurate and up-to-date records of young adults' progress and report any concerns to senior staff.

**Requirements**:

- A minimum of 1 year experience working with young adults in a similar setting.
- A relevant qualification in social care, health, or a related field.
- A good understanding of the issues facing young adults with a range of needs, including teenage parents, learning difficulties, mental health issues, and personality disorders.
- Excellent communication and interpersonal skills.
- The ability to work as part of a team and use your own initiative.
- The ability to work flexible hours, including weekends and evenings.
- A commitment to promoting the welfare and well-being of young adults.

**Job Type**: Temporary contract
Contract length: 6 months

**Salary**: £11.00-£14.91 per hour

Schedule:

- 10 hour shift
- 12 hour shift
- 8 hour shift
- Day shift
- Every weekend
- Monday to Friday
- Night shift
- Overtime
- Weekend availability

Ability to commute/relocate:

- Croydon: reliably commute or plan to relocate before starting work (preferred)

Work Location: In person
