**Location**: Pembrokeshire

Pembrokeshire Housing services have been developed in partnership with Pembrokeshire County Council, Pembrokeshire Housing Support Grant Team and Hywel Dda Health Board. Adferiad Recovery provides a range of services to adults and Carers of those who are experiencing a mental illness. The support is to provide independent living domiciliary care, supported accommodation, group work, and/or 1:1 floating support working out in the community and in people’s homes. The Pembrokeshire supported accommodation projects include a 24 hour shared supported accommodation to six occupants who are diagnosed as experiencing a serious mental illness and have been assessed as needing high levels of support to help with their recovery.

**Senior Job Role**:
The position of Senior involves taking responsibility of delegated tasks such as the completion of processing referrals from point of receipt to the delivery of support through the creation of person-centred/housing related support delivery plans, allocating tasks to staff members. The role will include acting as Deputy in the Service Managers absence, completing reviews of support, recording evidence of outcomes for monitoring, ensuring all client files are up to date and ready for any validation, and ensuring compliance with internal and external quality standards inspection. A senior is expected to be able to take on any essential admin and oversee the management of Housing Support Workers and the projects as directed by the Service Manager.

**Duties and Responsibilities**:

- Supporting the Service Manager in processing of referrals - following procedures for completing new assessments, risk assessments and creating individual person-centred service delivery plans.
- Delivering support and assisting clients with the following:

- Feeling secure within their home
- Increasing independent living skills
- Using appliances safely and maintaining reasonable domestic standards
- Personal finance, budgeting, and accessing benefits.
- Mental and physical healthcare
- Self-management of medication
- Attending appointments
- Visitors and their access to individual’s property
- Building and maintaining relationships with neighbours, families, friends, professionals
- Developing skills and interests
- Attendance at multiagency meetings to ensure the complex needs of the client(s) are addressed appropriately.
- Support the Service Manager and Registered Manager, in monitoring and evaluating staff performance during supervisions and with ongoing support.
- Data collection and appropriate recording and presentation for monitoring and Quality Assurance reports as directed by the Service Manager.

**Person Specification**:

- NVQ or QCF Competencies in Health & Social Care Level 3 - all staff would be required to hold or complete this as part of their employment.
- Must have sufficient experience of providing care and support services to people with a serious mental illness and co-occurring substance use disorder. **OR**:

- Must have sufficient experience of providing care and support services within a social care setting.
