Job Overview:
We are seeking a Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing administrative support and coordinating care services for our clients. This is a rewarding role that requires excellent organisational skills and the ability to work in a fast-paced environment. This a Part Time role.
**Responsibilities**:
- Coordinate care services for clients, ensuring their needs are met
- Schedule and coordinate appointments with healthcare professionals
- Maintain accurate and up-to-date client records
- Communicate with clients, their families, and healthcare providers to ensure effective care coordination
- Assist in the development and implementation of care plans
- Monitor client progress and make necessary adjustments to care plans
- Provide administrative support, including managing documentation, filing, and data entry
- Collaborate with other members of the healthcare team to ensure seamless care delivery
**Requirements**:
- Previous experience in an office or administrative role is essential
- Excellent organisational skills with the ability to multitask and prioritise workload
- Strong attention to detail and accuracy in record keeping
- Proficient in using Microsoft Office Suite (Word, Excel, Outlook)
- Excellent communication skills, both written and verbal
- Ability to work effectively in a team environment
- Compassionate and empathetic nature
