We are recruiting a Review Assessment Officer who will complement our team. You will be one of the main points of contact for people within the service, their family or chosen representative, helping them achieve the best possible therapy/care goals and be part of their journey in returning home, following an often difficult and stressful time of being unwell.

This is your opportunity to form positive relationship with people to encourage wellbeing whilst assessing their needs and adapting care interventions to encourage independence. You will find this a pleasurable and rewarding job but not without its challenges in the best possible way.

No one day will be the same, you will support the team to deliver personal care with an enabling approach, work with people in service, work alongside pharmacy, nurses, therapy colleagues and the daily GP to collaboratively devise support plans that are strength based and person centred and tailored to meet the individual person’s needs and goals.

We really need you to be a strong, influential character who can use your initiative to support, guide and supervise a team of enthusiastic and caring staff to deliver a range of interventions. You will be working alongside likeminded inspirational professionals to continually shape and develop the service.

There is no doubt that this role will offer you a varied opportunity to be part of Multi-Disciplinary Meetings, carry out social care assessments for onward discharge, draw up Risk Assessments, Support Plans, Medication Administration records, Reviews and much, much more

As a Review Assessment Officer you will be very much part of the wider Adult Social Care Team as well as working with a range of local community services offering you the opportunity to enhance a person’s quality of life by linking them in with Integrated Neighbourhood Teams to help them to achieve independence and remain at home for as long as possible.

- Have passion, drive and vision for the future of Intermediate Care
- Have an engaging style that can Lead and motivate staff
- The ability to achieve multi-disciplinary team results through dedication for rehabillation, evidence based knowledge and caring nature
- The ability to lead and support the direction of care on a day to day basis through flexible working patterns. Including monitoring of rotas and staff competnaices
- Have proficient skills in using ICT systems and Microsoft.

An NVQ level 3 in Health and Social Care is essential and willing to work towards a level 4.
