**Job Purpose**:
The Dyfodol Offender Interventions Service is an integrated partnership provision for South Wales and for the public sector prisons. The aim of the service is to address the causes of drug and/or alcohol related offending and risk of harm to individuals, families, peers and their communities through assessment, engagement and diverting individual offenders out of crime and into appropriate levels of interventions. We are looking for a Prison Caseworker to develop and sustain close working relationships with a number of organisations to align to and support the work of the Area Planning Boards. You will also provide support for adult offenders and prisoners who have a drug and /or alcohol dependency who reside within the local authority area of Cardiff.
**Main Responsibilities**:
- Provide advice, information, support, and interventions in relation to substance misuse and signposting to appropriate services.
- Provide a range of accurate and relevant information on drugs and alcohol and their effects to prisoners and to professionals within prisons.
- Provide substance misuse assessment, case management and care planning.
- Facilitate group work sessions alongside prison staff and healthcare teams.
- Reduce the risk of overdose by providing advice and information to reduce relapse and deliver Naloxone/ Prenoxad training prior to release.
- Maintain accurate client records and collection of information for monitoring and evaluating effectiveness and quality of service.
- Liaise with appropriate services to ensure continuity of treatment and support, for those receiving treatment prior to custody, during custody, on transfer, and on return to the community; including being the main lead for substance misuse whilst the prisoner is in custody.
**Person Specification**:
- NVQ 3 in Health & Social Care and/or relevant professional qualification.
- Experience of building productive working relationships with a range of partners.
- Experience of working in multi-disciplinary and multi-agency teams.
- Experience of working towards achieving individual and team objectives.
