**Job Title**: Social Prescribing Link Worker (SPLW)
**Hours of work**:37.5
**Salary**: £31,500
**Contract**: 24-months
**Location**: Brompton PCN, this role will be on-site 5-days a week
**Job Scope**
Social prescribing empowers individuals to manage their health and wellbeing by connecting them with non-medical 'link workers.' These workers focus on what matters to the individual, taking a holistic approach to link people to community groups and services for practical and emotional support. Social prescribing strengthens community and personal resilience, reduces health inequalities, and addresses broader health determinants like debt, poor housing, and physical inactivity by increasing community involvement. It is particularly beneficial for those with long-term conditions, mental health issues, loneliness, or complex social needs affecting their wellbeing.
**Key Responsibilities/Tasks**
**Promote Social Prescribing and Build Relationships**:
- Promote social prescribing and its role in self-management.
- Build strong relationships with key staff in GP practices and local agencies within the Primary Care Network (PCN).
- Attend relevant meetings, provide information, and offer feedback on social prescribing.
- Encourage referrals and raise awareness of the benefits of social prescribing.
- Proactively encourage self-referrals, especially from hard-to-reach communities.
**Provide Personalised Support**:
- Meet individuals one-on-one, including home visits, to understand their needs and build trust.
- Use a holistic, strength-based approach to develop personalised support plans.
- Address wider health determinants such as debt, poor housing, unemployment, loneliness, and caregiving responsibilities.
- Connect individuals to community groups and services, ensuring they feel comfortable and supported.
- Follow up to ensure engagement and satisfaction.
- Assist with exploring personal health budgets for additional support.
- Support and forge strong links with local VCSE organisations and community groups.
- Utilise networks to create a community asset map.
- Promote funding opportunities like micro-commissioning or small grants.
- Ensure referred groups have safeguarding, health, and safety procedures in place.
- Facilitate compliance with information governance policies and the Data Protection Act.
- Sustain community groups and partnerships.
- Collaborate with local partners and commissioners to identify unmet community needs.
- Encourage volunteering and peer support to strengthen community resilience.
- Develop a team of volunteers within your service to provide ‘buddying support’ for people, starting new groups and finding creative community solutions to local issues.
- Conduct regular ‘confidence surveys’ to community groups receiving referrals to ensure all are well-supported and sustainable.
**Data Capture**
- Work sensitively with individuals, families, and carers to gather information and track the impact of social prescribing on health and wellbeing.
- Encourage sharing of feedback and personal stories about the impact of social prescribing.
- Support referral agencies in providing accurate information and use case management systems to monitor progress.
- Provide feedback to referral agencies regarding referred individuals.
- Collaborate with GP practices within the PCN to ensure social prescribing referral codes are entered into SystmOne and NHS usage is tracked, complying with data protection laws and agreements with the integrated care board (ICB).
- Use the Joy platform to record caseload.
**Clinical Governance**
- Identify risks that impact on peoples’ health or social care needs.
- Report all accidents/incidents, and all ill health, failings in equipment and/or environment to line managers.
- Contribute towards audit and data collection as required.
- Once assessed as competent will be accountable for their own practice within their area of responsibility when identified and agreed with the line manager.
- Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the roles and responsibilities.
- Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
- Work with the Clinical Director to access regular ‘clinical supervision’, to enable you to deal effectively with the difficult issues that people present.
**Miscellaneous**
- Work as part of the team to seek feedback, continually improve the service and contribute to business planning.
- Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner.
