The post holder will work as part of the Cardiac rehabilitation service at North Manchester General Hospital, which is jointly run with specialist Cardiology Specialist Nurses and physiotherapists.

The post holder will undertake highly skilled support work for people who have cardiac disorders and complex multi pathological conditions. The work will involve assisting the Cardiac Rehabilitation nurses and therapists in supporting the screening, assessing, and goal setting of cardiac patients and the delivery of Cardiac rehabilitation exercise and education programme, adapting the programme to meet individual patient needs.

Work will be carried out in both the hospital and community setting.

The post also includes specific administration tasks including audit to support the daily running of the department.

The post-holder will undertake administrative duties including scheduling of appointments and classes, data inputting for the National Audit of Cardiac Rehabilitation.

Due to the nature of the working environment the post holder must be flexible and adaptable during their working day to ensure the needs of the service is met. All staff are expected to work to the Trust Values

Effectively plan, prioritise, and manage their own workload.

Prepare treatment areas.

Establishing and maintaining appropriate working relationships with colleagues, patients and carers and other professionals encountered in the course of your duties.

Obtain informed consent prior to undertaking any assessment or intervention.

Participate in the assessment of any risk associated with delivering the service, minimising risks where possible.

Monitor patients and promptly alert a qualified practitioner when there are unexpected changes.

Monitoring the progress of patients and adjust interventions where necessary liaising with registered practitioners as required.

Participate in the planning and delivery of physical activity programmes for patients with, myocardial infarction, pre and post cardiac surgery, heart failure and PCI.

Participate in conducting functional assessments of cardiovascular patients, maintaining patient safety at all time.

Supervise and demonstrate exercises within cardiac rehabilitation sessions.

Ensure adequate levels of supervision of all rehabilitation sessions in line with service policies and procedures and patients own care plan.

Ensure that all patients’ progress through the relevant stages of the programme at the correct time and that appropriate information and advice is provided at each stage.

Assume responsibility for monitoring the progress of all patients undergoing cardiovascular rehabilitation evaluating their condition and adapting their programme as necessary within department standards and protocols seeking senior assistance as required.

Maintain all records associated with cardiovascular rehabilitation in an organised and confidential manner in accordance with the code of conduct.

Support the delivery of educational sessions on physical activity as part of cardiac rehabilitation and promote long term concordance to physical activity.
