Band 6 Learning Disabilities Clinical Nurse Specialist - Vulnerabilities

Full time 37:5 hrs Monday to Friday (0800-1600) Fixed term/secondment 2 years

An exciting opportunity has arisen for a Registered Nurse Learning Disabilities to join the Vulnerabilities Team. You will form part of the Vulnerabilities Team, who are a passionate and dedicated team of Learning Disability, Mental Health, General nurses and Dementia Specialists Nurses; You must be an enthusiastic, passionate and motivated Nurse who has transferable skills and expertise. To work as part of the team in shaping and transforming acute hospital care and experience for patients with a Learning Disability, and or Autism, Mental Health Conditions, Dementia, or for those who have sensory impairment or loss.

Your case load will be diverse as we work collaboratively to meet the health needs of this group of individuals attending the hospital, either as an inpatient or outpatient to ensure that their holistic care needs are met, by providing and supporting specialist intervention, adopting a person centered approach to optimize health and wellbeing, strive to address health inequalities and empower individuals to have independence, choice and control.

The post requires a flexible approach and requires the individual to work across sites depending on service demand/Pressure therefore car driver is essential.

**Main duties, tasks & skills required**:
The role is diverse; and you will be required to review and assess patients, liaise, support and teach staff, implement new ways of working, support reasonable adjustments, work collaboratively with individuals, families, carers and a wide range of Healthcare professionals when planning admissions, during inpatient care through to discharge to ensure a positive experience.

You will be required to work Trust wide/ across all sites, acting as a role model, sharing expertise across multi-disciplinary teams. Therefore, you will need excellent communication, organizational and interpersonal skills, you will need to be able to work as part of a team or on your own. You will need to be resilient to an ever changing workload be able to prioritize and manage the workload, and ensure a high standard of clinical care is delivered to the patients under our care

We welcome applicants who have a current NMC registration RGN/ RSCN RNLD or RNMH, with knowledge and experience of working with Adults children and young people with learning disabilities/ mental health conditions/Dementia or communication difficulties and or and complex needs

There are 4 main inpatient sites across the Trust and you will be expected to travel between sites, therefore a car driver is essential.
