We are currently seeking a Prison Recovery Coordinator within HMP Durham.

This is a Full Time post Monday to Friday 09:00 - 17:00

You must have Prison Vetting
- Manage an agreed service user caseload, reviewing on-going care and treatment and liaising closely with the full range of partners within the multi-agency Reconnected to Health team, as well as family members and significant others when appropriate.
- Work in partnership to carry out assessments, key working and interventions to enable individual service user progression from engagement, through structured treatment to sustained recovery.
- Facilitate service user access to treatment, assets and resources to increase recovery and social capital, supporting service users and their loved ones to achieve and sustain recovery goals.
- To exercise excellent communication skills and therapeutic skills when dealing with service users, carers/families and partner agencies.
- To work closely and collaboratively within the multi-disciplinary team across the Reconnected to Health partnership and externally, providing advice and information to managers and other colleagues, through attendance at reviews, team meetings and via telephone conversations and MIS information.
- To provide effective coordination of individual service user’s recovery plans and actively promote service user and stakeholder involvement within recovery planning.
- To improve outcomes for children and families of service users by reducing the impact of drug and alcohol related harm on family life and promote positive family involvement in recovery.
- To lead effective case management for allocated service users, including on-going processes such as risk assessments, risk management plans and safeguarding assessments and plans.
- To develop and deliver quality psychosocial interventions that meet local need across both group work and one to one formats.
- To co-ordinate delivery of group work sessions and programmes including development of resources ensuring local needs are met, provision is evidenced based and sessions are engaging.
- To support service users to positive transition within the prison system through, where possible, planned preparation for release or transfer that promotes and protects sustaining recovery capital and reduces duplication and gaps in care. This will include effective partnership working with a range of community and prison-based services.
- To promote and enable engagement with the full range of health and wellbeing opportunities within Reconnected to Health, in particular in relation to mental health, smoking cessation, pain management and blood borne virus testing, treatment and vaccination.
- To collaboratively plan and deliver shared assessments, interventions and reviews with Reconnected to Health colleagues, promoting seamless care for service users.
- To promote visible recovery across the service and ensure peer support opportunities are relevant and accessible for all service users.
