This post is for a navigator to be part of a multi-disciplinary team within the YOUnited Referral Hub. You will contribute under the close supervision of qualified staff and the post will have varied responsibilities.

To work alongside colleagues to support the processing of referrals and consultation with children, families and professionals requesting emotional wellbeing and mental health support for Children and Young people across Cambridgeshire and Peterborough.

To undertake liaison work with referring agencies and stakeholders to improve understanding of the referral pathway, quality of referrals, and to support navigation of the referral process into YOUnited.

1. Work alongside colleagues to support the processing of referrals and consultation with children, families and professionals requesting emotional wellbeing and mental health support for Children and Young people across Cambridgeshire and Peterborough.
2. To undertake liaison work with referring agencies and stakeholders to improve understanding of the referral pathway, quality of referrals, and to support navigation of the referral process.
3. To foster positive relationships with key stakeholders and support collaborative working and improved signposting to external providers.
4. Assist in the gathering of information and analysis from clinical records.
5. Participate in the multi-disciplinary daily meetings to support the clinical effectiveness of the referral hub as required.
6. Support YOUnited colleagues with internal and external training and group work.
7. Assist in clinically related administration, conducting audits, collecting data and statistics, development of audit and/or research projects.
