**Job Profile**

**Job Title**: **SUPPORT WORKER**

**Hours**:42 Hours per week

**Reports to**:Team Leader

**Summary of Role**:
**Carry out a range of shift duties within the day-to-day programme of support and resettlement. In so doing, affording care and comfort to the personal needs and safety of residents as required, respecting the dignity of the individual and promoting independence.**

**Main Tasks**:
**1*. Assist in setting up individual programmes for them in regards to teaching life skills i.e. **, *budgeting, cooking, minor maintenance, filling in forms and writing letters, safety in the home, employment training, housing advice, accessing other support services,
DIY, buying furniture, reading meters and claiming Community Care Grants etc.

**2. ** Help to create a relaxed, homely attractive and personal living environment which maximises the individual residents opportunities for self-determination and to relate to the resident’s individually and as a group, meeting their physical, intellectual, emotional and social needs in sympathy with the ethos of the organisation. To promote a greater level of independent living within the hostel the independent flats, adjacent to the hostel and the community.

**3.** Help ensure that residents are given the opportunity to enhance their quality of life, that they retain their citizen rights and are encouraged in the taking of decisions in all matters which affect their lives.

**4**. Help ensure that the needs and wishes of all residents are ascertained, respected and met wherever possible, particularly bearing in mind ethnic, religious and cultural factors.

**5.** Protect residents rights and choices and prevent them from becoming institutionalised.

**6.** Encourage continuity for residents in terms of maintaining links with their former life, where appropraite and help develop support within the greater community if required.

**7.** Ensure that the needs of the residents such as independence, privacy and dignity are respected and met where possible.

**8.** Maintain and create links with others who are involved in the support and care of the residents eg. Doctors, nurses, family also other services in the community such as libraries, clubs and community centres.

**9.** Ensure that the needs of residents are paramount.

**10.** Share in activities with residents such as reading, writing, hobbies and recreation.

**11.** Undertake the keyworking of a number of residents on a regular basis, recording details, maintaining records and updating our IT system as well as the Team Leader on progress.

**12.** Monitor arrears and support residents in this area.

**13. **Carry out dining room duties, as required.

**14. **Under the direction of the designated manager to distribute medicines in line with policy.

**General**

**16.** Provide written and oral information for resident reviews and to attend reviews as required.

**17.** Maintain written and computerised records as required by the Hostel’s policies and procedures.

**18**. Undertake training as necessary and to attend staff meetings and Service User’s meetings as requested.

**19.** Observe health and safety policies and procedures and report any matters of concern to the Team Leader.

**20.** Maintain and have an understanding of confidentiality.

**21.** Ensure all quality and assurance policies are adhered to.

**22.** Receive and show visitors around the project.

**23**. Undertake night duty as required.

**24.** Develop good relationships with other organisations and agencies and undertake presentations and provide information as appropriate.

**Team Work**:
**25.** Work with other members of staff within both resettlement and support.

**26**. Contribute to shift requirements in the event of staff absences.

**27.** Contribute to good communication within both the Resettlement and Support team.

**28**. Undertake other duties as may reasonably be required by the HSM.

- NVQ Level 2 Health and Social Care qualification or equivalent
