We have an exciting opportunity for a Health and Wellbeing Coach to join our team based within Primary Care within the South Lincolnshire Rural Primary Care Network. The development of Integrated Neighbourhood working is supporting providers to work in a joined-up way. Primary Care play a key role in the success of Integrated Neighbourhood working, supporting patients to self-manage their condition or be assisted to manage their health needs closer to home, in their local communities. This approach enables more people with health and social care complexities to achieve a greater balance in all round health.

**South Lincolnshire Rural Primary Care Network includes the following practices; Gosberton Medical Centre, Moulton Medical Centre, Sutterton Surgery, Galletly Practice, Abbeyview Surgery, Holbeach Medical Centre, Littlebury Medical Centre, The Suttons Medical Group and The Deepings Practice**

Health and Wellbeing Coaches work as a key part of the primary care network (PCN) multi-disciplinary team, as well as an integral part of their assigned Practice. They consider the whole person in addressing existing issues and encouraging the proactive prevention of new illnesses, promoting lifestyle approaches. Health coaching revolves around using coaching skills to support people with lower levels of patient activation to develop the knowledge, skills and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise support offers.

The post holders will work in partnership with their Practice clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients. The role is varied, and may include supporting self-management education, peer support and case management. You will take an approach that is non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions. As a whole your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long term conditions.

We are currently looking to recruit a couple of coaches to deliver against a no. of specific projects and who have training and/or experience in delivering:
1. Strength and balance group sessions (preventing falls) in an effort to prevent falls from happening,

2. Combatting loneliness - set up and deliver a befriending service - major problem for many for various reasons. They then focus on aches/pains and become depressed/low etc. - consequences little or no quality of life and increase in calls to Practices.

3. Bereavement support - waiting lists too long, many patients have not had emotional support 1-2 years after their loss which is negatively affecting their ability to move on - consequences of this is many calls to Practices.

**Key responsibilities**

1. Approach patients from Practice related data (using various clinical systems) in specified areas and the wider practice team including GP’s, Practice Nurses, Social Prescribers, Clinical Pharmacists and Physiotherapists who will have identified patients that meet the criteria for working with Health and Wellbeing Coaches.

2. Coach and motivate patients through multiple sessions to identify their needs, set goals and support them to implement and achieve their personalised health and wellbeing objectives, by providing them with advice, guidance and a management plan, personalised to their individual needs. It could include the national screening programme, chronic disease management, dietetics and healthy eating, lifestyle medicine, getting active and personal coaching and motivation.

3. Work with patients encouraging them to take responsibility for their health and wellbeing - specifically in the prevention of developing further illness/harm, need for secondary care, or the deterioration of existing long term health conditions.

4. To be part of our practice Health and Wellbeing team and to work alongside our existing Social Prescribing Link Workers enabling referral of patients into Social Prescribing once their current intervention has been achieved or is appropriate.

5. Ensure all interventions and coaching are designed to empower patients to be active participants in their own healthcare, empower them to manage their own health and wellbeing, and live independently.

6. Work with the broader MDT to maximise the support available to patients, including the social prescribing team to connect patients to community-based activities which support them to take increased control of their health and wellbeing, and working with clinical colleagues to provide enhanced support to patients being supported through, identifying those who wou
