**Primary Care Mental Health Clinical lead B7: West Penwith Primary Care Network (PCN)**

Here in West Penwith we are creating a brand-new model as part of the transformation of mental health care in Cornwall. There has never been a more exciting time to work in mental health services, especially in primary care. We want to hear from people who have passion and creativity to help shape and build our new, holistic service in Penzance. We aim to create a space where we understand ‘what matters to someone’ not ‘what is the matter with someone’. Our new team will include a diverse range of people all there to listen, triage and hopefully help with aspects of a person’s life that may adversely affect their mental wellbeing.

We are currently underway in creating a welcoming hub in Penzance, St Ives and Hayle. We are searching for someone to help us realise this for our patients. Being able to sit with a certain level of uncertainty is a must as we plan our new service

Although employed by Cornwall Foundation Trust jointly with the Primary Care Network, this role will be hub based and you will be a valuable member of the primary care network for the GP practices. You will benefit from regular supervision and hopefully love where you work, join us, let’s work together

**Main duties of the job**:
**West Penwith Primary Care Network**

The post holder will be based and embedded within a Primary Care Network (PCN) working with GPs, practice nurses and other Additional Roles Reimbursement Scheme (ARRS) workers e.g., Mental health practitioner, social prescribers, health and wellbeing coaches, care co-ordinators, as part of the wider primary care Multidisciplinary Team (MDT). The hub model will also involve many other valuable services from the voluntary and community colleagues. They will work with primary care GPs to manage the mental health needs of their population at the point of contact. This includes those with lower-level support requirements that can be met within primary care.

The aims of this role will be to lead and develop the new service alongside other PCN leads, eventually creating three hubs across the PCN including Penzance, St Ives and Hayle.
