We are excited to announce that we are recruiting full time Band 4 Nursing Associates to join our Community Integrated Response Team (CIRT).

**CIRT consists of 3 teams**:
Home First Team - supporting people to return home after a stay in hospital and continue their recovery and rehabilitation in their own surroundings

Urgent Community Response (UCR) - Providing people with urgent assessments, care and treatment for a short time at home, preventing unnecessary hospital admissions.

Virtual Frailty Wards - Providing a safe alternative to hospital for patients living with frailty in the community.

You must have the Nursing Associate Foundation degree and be on the NMC Register.

You will provide a professional service to patients under the Home First division of CIRT, using your knowledge and experience to make evidence based decisions and solve problems.

A warm welcome awaits the successful applicant. The Home First team is committed to driving forward patient care to achieve positive outcomes for all.

Home First was launched in October 2022 and has supported hundreds of patients home safely. Due to this the service has expanded and is now looking for a Nurse Associate to support more complex patients home.

We are an enthusiastic, energetic team with a 'can do' attitude and a great sense of team spirit. We support each other and are looking for someone with that same spirit who to join us.
