**Job summary**

**W*ickhambrook, **G**lemsford, **G**uildhall, **L**ong **M**elford **P**rimary **C**are **N*etwork are responsible for providing clinical and non-clinical support to patients across Suffolk.

The main duties of the Care Coordinator is to support surgeries across the PCN with the collation of information on people requiring targeted support from a range of clinical and non-clinical colleagues. The successful post holder will provide coordination, secretarial and administrative support across the PCN.

Post holders will be the first point of contact for staff, member practices and will be able to signpost queries to relevant individuals as appropriate.

You must have excellent interpersonal and communication, IT/data management, skills as well as experience of working with System One and other NHS systems.

**Job Purpose**

Working across the PCN the Care Coordinators will support surgery teams by liaising with patients, booking appointments, gathering data, recording and monitoring progress of activity, arranging meetings, taking minutes and action notes.

**Job Types**: Full-time, Part-time, Permanent, Temporary contract
Part-time hours: 20 per week

**Salary**: From £22,816.00 per year
