Outreach Support Worker

Jump 2 Independence Ltd are recruiting an Outreach Support Worker to join our Outreach Community Team. Jump 2 Independence Ltd provide support to people with varied care needs, including people with learning disabilities, mental health needs, sensory/physical disabilities etc. We strive to provide support to people in a way that is respectful, promotes independence and enables individuals to realise their own potential. This may also include supporting individuals in accessing the community, in their own homes or in any setting their needs and choices may require.
**Contracted Hours / Shifts**:
- Salary based on working an average of 25-30 hours per week.
- Outreach operates Monday - Friday between 08:00 - 17:00, possibility of overtime during annual leave and sickness.
- Annual leave equivalent to 5.6 weeks.
**Duties**:
- Your role will include supporting people who live in the community within their own homes on a 1:1 or 2:1 basis, you will provide support which may include issuing medication, safe manual handling, meal preparation, cleaning tasks, accessing activities within the community shopping and assisting to appointments and any other tasks required as part of the service users care plan.
- Outreach service users have an allocated call time between 1 - 7 hours blocks, you could see only 1 service user during the day or have back to back calls, the rota will be issued in advance with days and time in accordance with your weekly contracted hours.
- You will be required to transport service users in your own vehicle with business insurance cover as part of your role.
- To liaise as needed with external organisations, including the local authority and medical professionals.
- To comply with all Policies of the Organisation, as appropriate.
- To ensure that the day-to-day Support Services are operated in accordance with Health & Safety legislation and current statutory regulation guidance on best practice.
- To conform to the Company Code of Professional Conduct.

**What you will need**:
- Diploma in Health & Social Care Level 2 or above, or be willing to work towards.
- Previous experience in care (Experience in Supported Living an advantage).
- Care experience or a passion to work in care as we provide a full induction and on-going training to support you within your role.
- Commitment to making a difference in the people's lives whom we provide a high standard of care and support for and the ability to provide hand's on personal care in way that is dignified and respectful.
- Good written and verbal communication skills.
- Full driving licence and own car is essential due to the locations of some our calls.
