This is an exciting opportunity for a Nursing Associate or Assistant Practitioner to work in a team delivering person centred support to our local population with the aim of improving health equity and reducing inequalities. You will be working under the supervision of the Clinical Outreach Lead practitioner and the Primary Care Network Health Inequalities Lead and in collaboration with the wider Integrated Care Community (ICC) with the aim of delivery enhanced health checks to improve health outcomes. This is a pilot project funded by population health and will be delivered on a PCN footprint to include all patients within Lancaster PCN.

This vacancy is for a fixed term until the end of March 2025.
