An exciting opportunity has arisen to join a fast paced and vibrant administrative team. Supporting across both sites St Richards and Worthing.
- Attendance at daily board rounds on each ward on caseload.
- Attendance at weekly MDT (multi-disciplinary team) meetings on each ward in caseload.
- Participate in twice daily Touchpoint Calls when covering the Discharge Hub and communicate outcomes and actions to relevant parties.
- Communication with all internal staff (including ward staff, senior managers, immediate colleagues, representatives from other departments etc), external organisations, patients, relatives/friends and the general public.
- Communicate sensitive information which may cause upset or anger, and manage this situation.
- Communicate complex information in an understandable manner.
- Lead on discussions regarding discharge planning with MDT professionals and patients/representatives.
- Ensure hand-over of caseload to colleagues is accurate and complete.
- Negotiate in disputes over care provision and/or funding to achieve optimum outcome for the patient and/or Trust.

The post holder will be responsible for the design, implementation and monitoring of failsafe procedures, which close the loop from GP referral, call and recall, exclusions and referral to the Speciality. The post holder will be responsible for the quality of the data and participate in regular data cleansing exercises and liaising with staff from other organisations involved in the programmes. You will have a base, with some cross-site travel required.
