An 18.5 hour case worker post has arisen within the Home First Hospital Team.

With appropriate supervision and support undertake initial and full assessments of individuals with less complex care needs.

With appropriate supervision and support develop a support plan that is person centred and outcome based and enables individuals to manage risk within the support options they chose.

Monitor and review support plans, identifying outcomes achieved and unmet needs and aspirations and, where required, assist in the continuing management of individual's support.

Facilitate links with the community, collating and share information about services and community resources to enable individuals to have choice to meet their individual needs.

Develop and maintain effective working relationships with colleagues and external stakeholders to ensure the smooth transition of individuals and their circle of support through health and social care system.

With appropriate supervision and support, and where appropriate, take the lead on the assessment of Continuing Health Care funding where appropriate.

Maintain case records, ensuring that all necessary information and data meets the relevant and stated levels of accuracy, quality and timeliness.

Act in accordance with national legislation and local policies and procedures in relation to The Care Act, The Mental Capacity Act, Deprivation of Liberty guidance and Safeguarding Vulnerable Adults.
