We are delighted to advertise an exciting opportunity for an aspiring trainee Advanced Clinical Practitioner to join our acute medical team at Bradford Teaching Hospitals Foundation Trust supporting our Ambulatory Emergency Care Unit, Acute Medical Unit and Virtual Services. At BTHFT we recognise that ACPs are an integral part of the NHS workforce in the 21 st century as they provide flexibility and new ways of working and we are looking to appoint a trainee ACP with a passion to providing the highest standard of clinical expertise and autonomous practice.

We are looking for a self-motivated, enthusiastic trainee practitioner with a desire to develop exceptional leadership skills and experience working in an acute medical and virtual setting. They will provide holistic, personalised care and will take pride in caring for our community of Bradford. The trainee ACP will link with members of a fantastic, well established and efficient team of ACPs providing strong leadership to members of the multi-disciplinary team. They will deliver quality care and a faster response to patient needs resulting in better outcomes

The post holder will be practising under supervision as a trainee within clinical placement areas to provide advanced patient-centred care. This will encompass the skills of assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the clinical placements.

The post holder will, under supervision, assist in the safe assessment, management, referral and discharge of patients with undifferentiated and undiagnosed presentations in any area of the clinical placement. Throughout the training programme the post holder will develop the skills of an autonomous practitioner ensuring that evidence based practice is inherent in as aspects of care and treatment.

On completion the practitioner will have demonstrated clinical expertise, competence, confidence and be able to work within an autonomous and defined sphere of practice, maintaining records and in collaboration with healthcare colleagues.

This training programme will equip the post holder with the skills and knowledge to utilise and extend their existing primary field of professional healthcare practice.

The validated training programme will create opportunities and forums to consolidate theory and practice at an advanced level using specialist knowledge and evidence based practice.

The Trainee ACP will develop skills in triage of new referrals within clinical placements independently or with medical colleagues and in making onward referrals, maintaining professional and legal responsibility and accountability for all aspects of their own work.

The trainee ACP will maintain and ensure provision of expert clinical care for the patients within a complex case load under their management, and that of other staff.

The trainee ACP will be instrumental in supporting redesign of patient flow and patient referrals to support an integrated health care model cross cutting Primary and Secondary care.

The trainee will demonstrate progression of clinical skills throughout the trainee programme and receive dynamic formative and summative feedback from their clinical supervisors.

On completion of the training programme the ACP’s skills acquisition will be characterised by demonstrating an increasing complexity and range of clinical tasks and procedures. The trainee will be able to demonstrate greater clinical, technical, scientific and analytical skills. They will be able to synthesis complex facts and situations which must be taken into account in planning and evaluating procedures which inform and validate diagnosis.
