To be responsible for delivering the agreed community focussed recovery care
plan under the supervision of qualified members of the Community Enablement
and Recovery Team (community and discharge team) To focus on the direct
needs and requirements of the service users, working across the boundaries of
care.

CLINICAL.
To ensure that the Service User understands and has a clear pathway of care
across sector/agency boundaries with key contact points/named individuals
To work effectively as a member of the team to engage service users in social
and leisure based activities in a variety of settings
To work with individuals to establish meaningful activities within a range of
activities
To work within the CERT team to prevent unnecessary admission and support
service users to remain within their home environment
To work within the CERT Team to support service users upon discharge from
hospital to enable them and ensure smooth transition back into their daily
routines
To provide practical support to service users in developing and managing dignity
and independence
To provide information on health promotion
To monitor service users progress, level of functioning and mental state - recording information and reporting to CERT qualified team members and care coordinator on a regular basis via MDT and RiO and alerting relevant staff when concerns arise.
To support and work closely with community based resources within the CPA framework, thus promoting service user rehabilitation.
