We have an exciting opportunity leading our new** Reach Service **as a** Team Leader.**
We are thrilled to present a dynamic role for a Team Leader who will spearhead the delivery of Inreach/Outreach Team services to key sites and hospitals across West Kent to offer harm reduction and promote access to treatment, playing a pivotal role in the transformation of lives.
The post holder will coordinate and lead a small team of Inreach/Outreach workers and ensure a high quality and consistent service is delivered to individuals and groups who traditionally and regularly find services challenging to access; the service users may have either drug and/or alcohol related issues and may have a number of unmet needs.

**Working Hours**: 37.5 per week
**Salary**:£34,700.09 - £36,635.38 pro rata (dependent on experience)
**Responsibilities**:
**About the Role**:
- To support a small team of Outreach Workers to deliver a service across key sites across West Kent promoting access to treatment for service users and support for concerned others.
- Operational line management of a team of staff which includes supervision (with reflective practice), appraisal, performance and sickness management as well as ensuring staff follow CGL and external policies applicable to their roles.
- Ensuring quality of service delivery through data analysis and engaging staff in service quality improvement planning. Where necessary, create team and individual performance plans to improve quality and performance.
- Empowering staff to promote hope, empowerment, choices, control and opportunities that assist service users reaching their full potential as individuals and community members.
- Work with your team and Designated Safeguarding Lead (if not your role) to ensure that the practices within the team/service enable identification of service users, children and families who are at risk. Ensure that staff are competent in the delivery of interventions that reduce risk to service users.
- To adhere to CGL policies and processes in the delivery of Inreach/Outreach services in particular with risk assessments for the health & safety of staff when lone working, remote working, offering home visits etc.
- Ensure the team delivers rapid and flexible interventions where required to meet the needs of the service users and partner agencies.
- Working with the management team, maintain strong governance and monitoring systems, ensuring a consistent, high quality service and ensure professional standards are met and best practice is implemented.
**About You**:
- Possess a degree or equivalent relevant experience.
- Possess knowledge and experience in developing and leading health and social care services, with a track record of delivering services that consistently meet high-performance and quality standards.
- Knowledge and experience of delivering services which evidence high performance and quality standards
- Experience of effective partnership working, relationship management, pathway and protocol development
- Excel in leadership, inspiration, and team development.
- Experience of delivering substance misuse Outreach services and experience of working within a hospital setting.
- Experience of working with individuals and groups who are hard to engage with and a good understanding of the principals of (MECC) Make Every Contact Matter.
- Proven experience of working in partnership with local agencies to develop packages of care for clients with a wide range of needs, such as mental health, physical heath, social care and criminal justice.
- Excellent knowledge of conducting risk assessments and developing health & safety plans
