Are you passionate about perinatal mental health? Are you a postgraduate Mental Health Nurse, Social Worker or Occupational Therapist that would like to find out more about working in this field.

To work as part of a team to manage, triage, and assess referrals that are made to the perinatal mental health service.

Undertake urgent and emergency assessments and on occasion routine initial holistic assessments.

To participate in multi-disciplinary and multi-agency working, being aware of colleagues' caseloads and providing support to others as necessary and contribute to the on-going development of the Leeds Perinatal Service.

To maintain comprehensive and up to date written records of assessments, CPA plans, evaluations and on-going process notes in line with Trust Policy and to share these with the MDT and external agencies as appropriate.

To ensure effective communication and collaboration with others through attendance at, and chairing of CPA meetings, team meetings, ward rounds, clinical meetings and meeting held by partner agencies aimed at addressing the welfare of the child.

To ensure the service user and their families are central to and fully involved in the plan of care and have copies of care plans. I

Imparting sensitive and complex information to service users and their families.

To be aware of and adhere to all professional and Trust Policies including the reporting of incidents and accidents, complaints procedures and the management of confidential information.

To work collaboratively and communicate effectively across the primary care secondary care and children’s social care agencies.

The service provides a range of care packages for women experiencing a mental illness in pregnancy and up to 2 years postnatally. This includes, though is not limited to, a specialist holistic assessment of need for mother and infant, group work, tailored 1:1 therapies and advice to a range of professionals on the care and management of women suffering from a significant mental illness or at risk of developing a significant mental illness in the perinatal period. We often work very intensively with mothers at time when their need is very acute. Some of our families have very complex social circumstances and this needs sensitive handling alongside the management of the mental illness and the ongoing safeguarding of mother and infant.

As a service we are committed to providing regular and good quality supervision to our staff. We offer a comprehensive induction package, and care co-ordinators have the opportunity to undertake specialist perinatal mental health training both internally and externally.

To work as a member of a multi-disciplinary community mental health team offering a comprehensive perinatal mental health service to women experiencing mental health difficulties.

This post is a city wide Leeds post and will involve the care and support of women suffering from acute moderate to severe perinatal mental. It includes caring form mothers and their infants either, discharged from the Yorkshire and Humber inpatient service for a minimum of 3 months and on occasion, up to one year post discharge, or mothers resident in Leeds who need specialist interventions in the community within the perinatal period to prevent hospital, admission.

On occasion the post holder may be required to support staff on the Mother and Baby inpatient setting.
