We are looking for an enthusiastic, experienced flexible dietitian to join our friendly Dietetic Department in Ealing community and be an integral part of the Diabetes Integrated Care Service in Ealing (DICE)

You will be employed by Central North West London Foundation Trust but will be working under the umbrella of Ealing Community Partners

Day to day you will work within the well established multi-disciplinary Diabetes Integrated Care Service in Ealing (DICE) and be operationally managed by our Diabetes matron. The MDT team includes Diabetes Consultant, Diabetes Nurse Consultant, Diabetes Specialist Nurses, other Advanced Diabetes Dietitians and Diabetes Specialist Podiatrists to provide a service for Ealing patients with complex Type 2 Diabetes. You will also see a few patients with Type 1 Diabetes who prefer to be seen in the community instead of secondary care

The post holder will be responsible for contributing to the development of the service, setting up and running community dietetic and MDT clinics,
providing group education sessions, home visits, training and contributing to the evaluation of the service. You will also be responsible for training and supervising dietetic students.

Applicants must have excellent communication and team working skills and extensive experience of working with complex Diabetes

The post holder will demonstrate understanding and commitment to provide patients with complex Type 2 Diabetes with patient centred and holistic care, aiming to prevent possible co-morbidities and adverse outcomes. Post holder will demonstrate that they have the attitudes and behaviours required to support people to make and sustain healthy lifestyle and behaviour changes.

The post holder will provide specialist clinics, contribute to structured education programmes for adults with diabetes and training programmes for health care professions and students.

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

The Diabetes Integrated Care Service in Ealing is a diverse multi-disciplinary team of Diabetes Specialist Dietitians, Diabetes Specialist Nurses and Diabetes nurse Consultant and matron and medical consultant that works in community to design care plans that meet the needs of people with Diabetes. The team serves patients registered with a GP in the Ealing borough. The Team aims to optimise health and wellbeing, reducing complications and prevent unscheduled hospital visits. The team also sees a small number of patients with Type 1 Diabetes who prefer not to go to secondary care

**Key Responsibilities**:
Clinical

To be a dietetic advanced specialist for diabetes for the diabetes service, working as an autonomous clinical practitioner through direct referral, comprehensive client centered assessments, specific interventions and evaluation of practice.

Demonstrate a high level of advanced clinical competence in diabetes management; based on in depth knowledge, research and evidence-based findings

Development and delivery of training, and supervision for other health care professionals including new staff to the team, primary care teams and students.

Be responsible and accountable for a caseload of new and complex patients for a limited time, liaising with all members of the multi-professional team and colleagues in primary care.

Deliver structured education and supported self-management programmes as appropriate

Provide evidence based clinical advice on diabetes management to people with diabetes, their carers and health care professionals.

To work to promote good practice in diabetes care locally by offering leadership, guidance, education, and mentorship to primary health care professionals and other dietitians.

Plan, review and support patients individually within an outpatient setting or domiciliary visit and also deliver structured patient education for people with complex needs and uncontrolled diabetes. Support will also be directed to patients who require further self-management education.

Support people with diabetes and their carers to achieve individualized goals and care planning to maintain their physical and psychological well-being, encouraging self-management.

Support the clinical lead to develop policies for diabetes care for Ealing.

Participate in clinical supervision within the Diabetes MDT and within Nutrition and Dietetics for personal learning and development as well as supporting the dietitians providing different services across Ealing Community Partners

Network with other Diabetes dietitians in CNWL and beyond
