**Clinical Lead - Band 7**

**Warminster/Westbury Community Team**

**37.5 Hours per week - 12 month Fixed Term maternity leave contract or when leave finishes**

Warminster Westbury Community Team are looking for a dynamic, skilled and knowledgeable professional interested in taking on the clinical lead role over the next 12 months. The Clinical lead role will give you the opportunity to use your clinical knowledge to support the team with complex patient management and focus on provision of high quality, evidence based patient care including the two hour crisis response and falls service.

We are seeking a Clinician with community experience who has the ability to be part of a Leadership team. You will be expected to lead with a team facing approach and develop quality improvement within the team. You will be required to deputise for the Community Team Manager as well as work alongside a number of GP practices and advanced clinical practitioners and consultants. The post-holder will provide a high level of support and expert advice. You will also be expected to contribute to national and locally agreed targets to reduce unnecessary hospital admissions and to facilitate earlier discharges from the surrounding hospitals.
