As the largest independent provider of NHS services, Practice Plus Group is delivering a new model of Integrated Urgent Care that reduces pressures on services and staff. By bringing together NHS 111 call centres, clinical assessment and other urgent care services, we enable patients to access the right care, when they need it. It’s better for everyone.

As a **Coordinator** at our OOH service, you’ll be a single point of contact and liaison for all members of this multidisciplinary team, as well as other internal and external services. You’ll provide coordination and support to ensure the smooth running of shifts - and respond swiftly and efficiently if there’s any breakdown in the service. This could include making comfort/safety calls to patients when required.

**This role can be offered as permanent or bank (0 hour contract).**

The **Salaried **post and we are looking for people to work as either:

- In hours Coordinator - Tuesday, Wednesday and Thursday 07:45 am-12 pm.
- Evening Coordinators - 5:00pm-11:15pm with a Mix of Weekends working 7am-4pm and 3:45pm - 11:00pm
- Overnight Coordination - 11:00pm 8:00am

- Pay**:£11.95 per hour** (Monday-Friday, 08:00-23:59); **£14.92 per hour** (Monday-Friday, 00:00-07:59); **£14.12** per hour (Weekends, 08:00-23:59); **£16.68** per hour (Weekends, 00:00-07:59).

- 25 days’ annual leave, increasing with service up to 28 days per year + bank holidays.
