**Adult Social Care Reassessment and Transformation Team - Full time / 12 months fixed-term contract**

Exciting opportunities have arisen in the Adult Social Care Service within Sefton Council. The Reassessment and Transformation team are seeking applicants for the position of Community Assessor. We require the successful applicant to have the skills, knowledge and enthusiasm to deliver effective case management.

As a community assessor, you will contribute to the operational delivery of an effective Adult Social Care service as part of a team responsible for promoting the welfare of vulnerable adults and continuously improving outcomes for individuals. You will manage a defined case load delivering high quality strengths-based Care Act assessments and outcome-focused support plans and reviews that address the needs of the individual.
