**The post holder will**:
Manage staff within the West Norfolk Home First Hub and urgent community response (UCR)team, including recruitment, performance, professional development and appraisal.

Lead the improvement of peoples’ experiences of discharge from hospital and across the West Norfolk admission avoidance teams, through redesign of existing services to reduce and eliminate (as far as possible) delays and implement new service developments to promote and embed the home first ethos. This includes establishing and imbedding working with colleagues so that the HFH works effectively as part of the Integrated Urgent and Emergency Care hub for West Norfolk.

Implement national and local initiatives across health and social care embedding these in operational practice and procedure.

Develop excellent working relationships with commissioners.

To provide leadership for West Norfolk Home First Hub for further development and expansion of Discharge to assess (D2A) and UCR across West Norfolk to ensure that a high quality, safe and cost-effective service are delivered.

To identify opportunities for new business, funding and income and propose to Operational Director. To be responsible for managing and leading performance improvement across the HFH and as required other portfolio services.

Support Acute Trusts Complex discharge team to be as effective as possible.

To take an active role in the implementation and delivery of CIP plans, service line reporting and income targets across the portfolio of services.

To assist and advise the OD on the development and implementation of strategies and policies.

To develop strong productive relationships with key external agencies including acute providers, independent care home and domiciliary agencies, ICBs, GPs, NHS Norfolk and District Councils.

Create and build networks to support and contribute to service developments.

We are looking for a talented manager to join our integrated management team in West Norfolk. We place high value on working collaboratively with our partners to ensure patients and service users receive the best experience possible. This role provides strategic and operational leadership to our adult health and social care teams and will lead on delivering effective discharge and urgent community response pathways.

Provide leadership to West Norfolk HFH and UCR NCC and NCHC staff. Provide support to the QEH complex discharge team.

Lead and coordinate service development initiatives.

Ensure that the support and develop staff and students takes place.

Contribute to the implementation of National initiatives.

Develop and maintain effective communication within the organisations and with external agencies which informs, promotes and influences positive change.

Work alongside other operational managers and professional leaders to further develop the integrated care pathway and deliver safe and effective discharge and admission avoidance that delivers the overall strategic plan for both organisations.

When required, represent NCH&C and NCC at local, regional and national events.

Work collaborative with Integrated Care Board (ICB), GPs and relevant acute and mental health Trusts to develop pathways of care to meet the needs of the West Norfolk population.

Lead regular communication to Home First Hub and UCR staff, keeping them informed of organisational changes and ensure that the appropriate feedback mechanisms are in place for staff to raise any queries.

Ensure clear and effective communication between QEH and system partners in relation to urgent and emergency care priorities.
