**Exercise Referral Instructor**
- Role Profile / Job Description_

**Role Title **:Exercise Referral Instructor / Professional**

**Responsible To **:Exercise Referral Scheme Co-ordinator

**Hours **:Full Time (37 Hours per week)

**Salary **:£21,589 per annum

**Primary **Purpose**:
The post holder will be responsible for delivering a local Exercise Referral Programme with an overall objective of contributing to the National Exercise Referral Scheme and improving the physical activity levels and health of the people in Wales. Working closely with the Exercise Referral Co-ordinator you will be required to, in your role of Exercise Referral Professional, educate encourage, support and signpost service users to the most appropriate form of physical activity.

You will review personal activity programmes, give initial consultations, fitness inductions, perform progress reviews and carry out pre/post assessments with customers who have been referred from their doctor or other health professional. The exercise referral scheme is about helping people who are most at risk to enjoy a more active lifestyle.

**Specialist **Accountabilities**:(broad parameters)**
- To provide consultations for clients and collect data to monitor progress ;
- To deliver a range of high-quality physical activity opportunities that are progressive and reflective of the needs of special populations in the local community ;
- To implement strict data and management protocols, including GDPR ;
- To assist in the evaluation of NERS by collection of accurate data ;
- To sign post clients to appropriate exist routes ;
- To support any new ERPs and ensure that they are aware of their role in the scheme ;
- If required to attend and represent the NERS Coordinator at the NERS quarterly meetings ;
- To ensure the development and dissemination of good practice in the delivery of exercise by researching evidence regarding exercise and certain medical conditions ;
- To provide cover for other exercise referral professionals where necessary ;
- Under the direction of the Coordinator, oversee the day-to-day administration tasks of the scheme, including**:Telephone communications;**:

- Distribution of appropriate mail;
- Booking appointments;
- Monitoring scheme attendances and client programmes; and,
- Ensuring client follow-up procedures are implemented.
- To collate information regarding the scheme and disseminate them to the co-ordinator.

- To attend team meetings when required and to participate where necessary ;
- Take action to ensure that customer comments are resolved, responding to customer feedback and making any adjustments for improved service as appropriate ;
- To be fully conversant and comply with the Health, Safety, Fire and Security regulations and procedures of our Leisure venues, including risk assessments

**Success Measures**:

- Levels of Customer Service and high percentage of customer satisfaction / feedback;
- Internal feedback mechanisms;
- Continued progress against KPI’s; and,
- PDR and obtainment of annual objectives

- **Knowledge, Experience**

**1. **At least two years’ experience within a Health and Fitness environment including the initiation and delivery of Health and Fitness services to the public and special populations. Essential

**2. **Be I.T. literate and be competent in the use of Microsoft Office and Social Media platforms, particularly the ability linked to all aspects of virtual session delivery, hosting and attending virtual meetings, delivering face to face and virtual assessments / consultations, and completing all office / home working responsibilities. - Essential

**3. **Experience in providing fitness instruction and designing programmes for clients - Essential

**4. **Co-ordination, development and planning of leisure activities within a wide range of environments including community/educational settings - Essential

**5. **Experience of working on a GP Exercise Referral Scheme or a Chronic condition rehabilitation scheme - Desirable
