**Trusted Assessor - Maternity Cover - August 2023 - May 2024**

**Working Pattern: Full Time - Maternity Cover - Possibility for some flexible working with possibility to move to part time at the end of the cover period after May 2024**

**Location: Home based with travel to hospital/care home locations and other community locations in Wiltshire.**

**Summary**

An exciting opportunity has arisen to join our Trusted Assessor service in, providing maternity cover until May 2024. Working for Wiltshire Care Partnership - the Wiltshire Care Provider representative body - you will work alongside health, social care and care providers to help people move from hospital to care services, between care services when needs change, or at the point when someone first needs to access care.

**About the role**

We are looking for 1FTE to work without team covering a period of maternity leave, to work with local care managers, Wiltshire Council and NHS colleagues ensuring we maintain our efficient and professional service during this time. The role of the Trusted Assessor is to undertake an independent and unbiased assessment to determine suitability and requirements on behalf of care providers. You will be making a significant contribution that enables timely discharges from hospital and smooth transitions for people at a sensitive and sometimes worrying time in their lives.

Some evening and weekend work may be required as appropriate to the discharge process being supported, but this is rare. This is at your own discretion and agreed with your manager.
