The post holder will:
- Carry out clinical assessments independently such as transfers, mobility, stairs, personal care, discharge planning.
- Carry out ongoing treatment plans, to review and amend in line with competencies.
- To order appropriate equipment to support discharge planning.
- To complete onward referrals to appropriate agencies.
- To demonstrate Physical ability to carry out therapeutic handling without direct supervision. There is a frequent requirement to exert moderate physical effort for several short periods during the day.
- Delegate and supervise Therapy Clinical Support Workers.
- To carry own caseload.
- To complete patient assessments and treatments in line with competencies
- To liaise with other members of the MDT, families, carers and other agencies.
- To issue equipment
- To refer to onward services.
