**Role**:
**Who You Are**
- Knowledgeable about the legal framework and responsibilities relevant to the requirements and location of the post.
- Capable of establishing working relationships with service users and actively engaging them through consultation, participation, and partnership.
- Primarily tasked with ensuring that children, young people and families have access to high-quality support services.
- Keeps effective records on the electronic systems, which is inclusive of plans, assessments, chronologies, and associated general notes.
- Expected to deliver strength-based, empower interventions to address emerging worries and ensure positive outcomes for children and young people.
- You will need to effectively manage complex cases, lead supportive plans for vulnerable children, and advise on bespoke packages and programmes.

**What the Job Involves**
- Taking whole case responsibility to provide high quality, outreach and home-based Early Help support to children and families.
- Delivering targeted support to children across various age ranges and their families to prevent the need for statutory services.
- Completing early help assessments, arranging Team Around the Family meetings and supporting families to improve outcomes for their children.
- Offering support on cases that become more complex and reach level 4, ensuring a smooth transition between early help and social care.
- Providing intensive and targeted support to vulnerable children and young people, including those with complex emotional and mental health problems.
- Assessing risk and meeting the requirements set out in the Early Help Hub Practice Standards and promoting the safeguarding of children and young people in accordance with statutory guidance and legislation.
- Attending regular supervision sessions with the Consultant Social Worker, providing evidence that work is progressing and that plans are improving outcomes for children and young people, and contributing to evaluation and development of services and new ideas.
- Maintaining an up-to-date knowledge through training, reading, and supervision.
- Work in a performance management framework to carry out duties to the highest standard of professional practice, consistent with all City Council’s policies, codes of practice, vision, aims, values, and principles.
