The post holder will work as part of a 0-19 Children’s Services team within Chesterfield for Derbyshire Community Health Services NHS Foundation Trust, delivering services to children and families as part of a Section 75 contract.

Provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme at a community, universal, targeted and specialist level. To be able to undertake face to face contacts on a daily basis to respond to client needs.

To deliver the public health role as a SCPHN Health Visitor working with communities and partner agencies and delivering the core contacts within the Healthy Child Programme.
- To provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme.
- To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
- To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
- To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
- To manage a team of health visiting support staff.
