As a Band 6 Junior sister at the Virtual Ward you will be expected to manage daily operations of caseload, triage referrals for admission, support staff with patient escalation. Troubleshoot, safety net and make clinical decisions to keep patients safe at home. We currently have a well established Adult virtual ward. You will be pivotal in supporting the implementation of expanding our current pathways and implementing new pathways in adult virtual wards. Please note this vacancy role requires face to face patient management as well as virtual management and therefore the team members will be expected to conduct face to face assessments in line with patient/ caseload needs. You will provide managerial leadership to the nursing team. In addition, you will further develop junior staff.

Virtual ward software monitoring of patients

Identifying and proactively onboarding patients on the virtual ward caseload providing in reach support to identify adults in our inpatient settings.

To work as part of a team and the wider MDT

Good wide range of assessment skills

Teaching and inducting of new starters and students

Being part of and leading on MDT meetings

Supporting with HR and rostering responsibilities

Being the shift lead in the absence of the band 7

Support with projects and service improvement

Develop long term condition management skills and knowledge

Face to face contact with patients to conduct initial assessment and base line observations.

Administration of IV antibiotics in patients homes

Being organised and a role model to others

Lead a team and co-ordinate the clinical area in the absence of the Clinical Service Lead / Service Delivery Manager and reinforce the values and standards of compassion, dignity and respect in care, leading the delivery of clinically excellent care

Ensuring the planning and delivery of care is aligned to professional values of care, compassion and respect, act as a change agent, developing clinically effective practice through the effective utilisation and integration of evidence based practice.

Support the Clinical Service Lead / Service Delivery Manager in ensuring a culture of protecting patients from harm and deprivation of basic rights ensuring a safe and effective service with continuous quality improvement.

Promote a clean and safe environment for staff, patients and visitors by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical governance, including risk management and critical incident reporting and root cause analysis.

Ensure a high standard of care and record keeping in accordance with Nursing and Midwifery Council, Health and Care Professions Council, national legislation and local standards.

Demonstrating safe and effective care including; timely administration of medicines; effective observation of patients and demonstrate effective communication with the multi-professional team regarding patient care.

Assess the risks involved in the care of patients and ensure utilisation of practices and protocols to minimise those risks, including safe use of medical devices. To monitor
