Hub Coordinator

We have a fantastic opportunity for our continuously expanding organisation for an experienced Hub coordinator to coordinate the support and activities within our day care services with knowledge and understanding of adults with learning disabilities, Autism and challenging behaviour. The successful applicant will over see the daily running's with the support of the Hub team leader. You will work closely with the Team leader, ensuring this remains a safe, fun environment for the rapidly expanding service we provide. Ensuring transport rotas are complete along with staffing rotas. There is a well established staffing team in post.
**Skill set**:
Excellent literacy skills.
Excellent communication skills.
Ability to work on own initiative.
To give guidance to Hub team leader, who will support with daily running's and lead the team ensuring the implementation of person centred activities.
Computer literate including basic knowledge of Microsoft Word.
A pro-active mindset, with a willingness to be flexible, agile & react positively to change.
Ability to develop strong working relationships with others.
Ability to work to Harmony care’s values.
Ability to participate in the on call rota.
**Experience of**:
Writing and reviewing person centred support plans of a high standard.
Writing and reviewing detailed risk assessments of a high standard.
Day to day running's of a day care service with the support of the team leader.
Planning and implementing of activities.
Managing staff.
Completing Person centred meetings with the service users and families of Harmony Care.
Completing MDT meetings.
